Identifying Best Practices for Social Determinants of Health Referrals in Community Pharmacies

“The goal of this project is to identify facilitators, barriers and best practice recommendations for providing Social Determinants of Health (SDOH) referrals for patients in community pharmacies.

The current literature has detailed the success of certain aspects of the SDOH service process including implementation of the screening tool. However, further research is needed to understand best practices for pharmacy team-provided referral and follow-up processes to support sustainable community pharmacy SDOH services. This was recently uncovered as a research priority area by the Pharmacy Quality Alliance and Patient Advocate Foundation.

I am most excited about hearing from community pharmacists about their experience providing SDOH services.”
