Impact of Targeted Outreach on Immunization Rates of Patients with Diabetes”

Brenden Pham is a pharmacy resident at Fred Meyer in Portland, OR.
“I am grateful to receive the APhA Foundation Incentive Grant and believe this project will help shine a light on gaps of care that pharmacists have the ability to fill.
This project stemmed from the importance of community pharmacists’ role in patient education and vaccine administration, as well as highlighting a population that is vulnerable to more severe complications from vaccine-preventable diseases. As the population with diabetes steadily increases, so will the number of patients with higher morbidity from conditions such as pneumonia and shingles. This study aims to benefit patients with diabetes by identifying where the gaps in care are and determining the effect targeted patient education has on closing those gaps.
I am looking forward to seeing what patient barriers are most prevalent as contributing factors in the current gaps of vaccine care for patients with diabetes and if there seems to be a trend in the gaps of care. Understanding this will help us provide targeted education to increase immunization rates and improve patient outcomes.”
