Retrospective Review of an Independent Pharmacist-led Diabetes Management and Education Program in a Medicaid Population
Brittany Bullock is a PGY1 community-based pharmacy resident with UNC Eshelman School of Pharmacy & Moose Pharmacy in Concord, NC.
“Community pharmacists are the most accessible healthcare providers. We have and continue to show our worth as part of the healthcare team by providing personalized, patient-centered care to bridge gaps in care and improve patient outcomes.
Diabetes continues to be a significant public health concern and community pharmacists are uniquely positioned to help patients manage their diabetes in ways that extend beyond traditional medication dispensing. My project proposal stems from my interest in helping patients with diabetes live healthier lives and recognizing the important role community pharmacists play in patient care.
I am honored to receive an Incentive Grant and am excited to evaluate the pharmacy's implementation of this service and its effect on patient outcomes. I look forward to using the results to continue to enhance and refine the pharmacy's process to ensure effective and impactful interventions.”
