Project IMPACT: Diabetes Prevention

National Diabetes Prevention Program

In 2017, APhA Foundation entered into a cooperative agreement with the Centers for Disease Control and Prevention (CDC) called DP17-1705, to build infrastructure within community pharmacies in order to expand access to the National Diabetes Prevention Program’s (National DPP) evidence-based lifestyle change program designed to prevent or delay the onset of type 2 diabetes among adults with prediabetes. Throughout this 6-year cooperative agreement the APhA Foundation worked with independent and chain pharmacies, schools of pharmacy, state departments of health, community-based organizations, and federally qualified health centers to improve access to CDC-recognized lifestyle change programs in underserved communities across the United States.

​​As a result of these collaborations, over 4,000 pharmacies are in a diabetes prevention network, with screening, testing, and referral access available in 47 states. The establishment of this network allowed for over 48,000 individuals to be screened and for those who qualified to be referred to a diabetes prevention program to receive support on their journey to prevent or delay the onset of type 2
diabetes.​

Umbrella Hub Arrangement

As part of its umbrella hub arrangement the APhA Foundation received approval from CMS as a recognized Medicare DPP provider. This unique opportunity opens the door for pharmacy providers to receive reimbursement for their services and directly improve patient care in the communities they serve. Participating in the APhA Foundation Umbrella Hub allows pharmacies to focus on delivery of the program to their patients and creates a financially sustainable model for community pharmacies in diabetes prevention efforts.

Team-Based Care Approach

Our approach enables care teams to efficiently provide personalized, team-based care to patients at risk for developing type 2 diabetes. Patients can become better informed and learn how to self-manage their prediabetes, resulting in improved health and reduced risk of major complications associated with diabetes, including kidney disease, amputations and blindness.
